This is a Notice of the Presidential declaration of a major disaster for Public Assistance Only for the State of ARIZONA (FEMA—1581—DR) , dated 02/17/2005.
*Incident:* Severe Storms and Flooding.
*Incident Period:* 12/28/2004 through 01/12/2005.
**DATES:**
*Effective Date:* 02/17/2005.
*Physical Loan Application Deadline Date:* 04/18/2005.

The following areas have been determined to be adversely affected by the disaster:
*Primary Counties:* COCONINO, GILA, MOHAVE, NAVAJO, YAVAPAI
*Tribal Nations:* Hopi Tribal Nation, The Portion of the Navajo Tribal Nation Within the State of Arizona
