*Incident Period:* 09/12/2008 and continuing.
*Effective Date:* 10/03/2008.
*Physical Loan Application Deadline Date:* 11/24/2008.
*EIDL Loan Application Deadline Date:* 06/23/2009.

Illinois:
Crawford, Lawrence, Wabash.
Kentucky:
Boone, Daviess, Gallatin, Hancock, Oldham.
Ohio:
Butler, Hamilton.
All other information in the original declaration remains unchanged.
