*Incident Period:* 09/14/2008 through 09/15/2008.
*Effective Date:* 10/27/2008.
*Physical Loan Application Deadline Date:* 12/08/2008.
*Economic Injury (EIDL) Loan Application Deadline Date:* 07/09/2009.
