*Incident:* Hurricane Omar.
*Incident Period:* 10/14/2008 through 10/16/2008.
*Effective Date:* 11/10/2008.
*Physical Loan Application Deadline Date:* 12/28/2008.
*Economic Injury (EIDL) Loan Application Deadline Date:* 07/29/2009.

All other information in the original declaration remains unchanged.
