*Incident:* Severe Storms, Tornadoes, Flooding, and Mudslides.
*Incident Period:* 05/03/2009 through 05/20/2009.
*Effective Date:* 06/09/2009.
*Physical Loan Application Deadline Date:* 07/28/2009.
*EIDL Loan Application Deadline Date:* 03/01/2010.

Contiguous Counties: (Economic Injury Loans Only):
Kentucky: Morgan.
All other information in the original declaration remains unchanged.
