*Incident:* Severe storms, flooding, and landslides.
*Incident Period:* 06/18/2009 through 07/08/2009.
*Effective Date:* 08/18/2009.
*Physical Loan Application Deadline Date:* 09/28/2009.
*Economic Injury (EIDL) Loan Application Deadline Date:* 04/30/2010.

All other information in the original declaration remains unchanged.
