*Incident Period:* 02/05/2010 through 02/11/2010.

*Effective Date:* 07/07/2010.
*Physical Loan Application Deadline Date:* 06/22/2010.
*Economic Injury (EIDL) Loan Application Deadline Date:* 01/24/2011.

*Primary Counties:* Grant, Mineral, Monongalia.
All other information in the original declaration remains unchanged.
