*Effective Date:* 06/17/2011.
*Physical Loan Application Deadline Date:* 07/05/2011.
*Economic Injury (EIDL) Loan Application Deadline Date:* 02/06/2012.

The notice of the President's major disaster declaration for Private Non-Profit organizations in the Commonwealth of Kentucky, dated 05/04/2011, is hereby amended to re-establish the incident period for this disaster as beginning 04/12/2011 and continuing through 05/20/2011.
All other information in the original declaration remains unchanged.
