*Effective Date:* 07/15/2011.
*Physical Loan Application Deadline Date:* 08/22/2011.
*Economic Injury (EIDL) Loan Application Deadline Date:* 03/23/2012.

The notice of the President's major disaster declaration for Private Non-Profit organizations in the State of Indiana, dated 06/23/2011, is hereby amended to establish the incident period for this disaster as beginning 04/19/2011 and continuing through 06/06/2011.
All other information in the original declaration remains unchanged.
