*Incident Period:* 03/03/2012 through 03/11/2012.
*Effective Date:* 05/04/2012.
*Physical Loan Application Deadline Date:* 06/18/2012.
*Economic Injury (EIDL) Loan Application Deadline Date:* 01/18/2013.

All other information in the original declaration remains unchanged.
