*Incident Period:* 10/29/2012 through 10/30/2012.
*Effective Date:* 01/29/2013.
*Physical Loan Application Deadline Date:* 03/04/2013.
*Economic Injury (EIDL) Loan Application Deadline Date:* 10/03/2013.
