*Incident Period:* 04/28/2014 through 05/06/2014.
*Effective Date:* 06/13/2014.
*Physical Loan Application Deadline Date:* 07/14/2014.
*Economic Injury (EIDL) Loan Application Deadline Date:* 02/16/2015.

The notice of the President's major disaster declaration for Private Non-Profit organizations in the State of Florida, dated 05/14/2014, is hereby amended to include the following areas as adversely affected by the disaster.
*Primary Counties:* Bay, Calhoun, Holmes, Washington, Jackson.
All other information in the original declaration remains unchanged.
