*Incident Period:* 12/06/2015 through 12/23/2015.
*Effective Date:* 03/21/2016.
*Physical Loan Application Deadline Date:* 04/18/2016.
*Economic Injury (EIDL) Loan Application Deadline Date:* 11/17/2016.
