*Incident Period:* 07/11/2016 through 07/12/2016.
*Effective Date:* 08/09/2016.
*Physical Loan Application Deadline Date:* 10/11/2016.
*Economic Injury (EIDL) Loan Application Deadline Date:* 05/09/2017.
