*Incident Period:* 10/07/2016 through 10/15/2016.
**EFFECTIVE DATE:**
12/19/2016.
*Physical Loan Application Deadline Date:* 01/17/2017.
*Economic Injury (EIDL) Loan Application Deadline Date:* 08/15/2017.

All other information in the original declaration remains unchanged.
