**AGENCY:**

Office of the Under Secretary of Defense for Personnel & Readiness (OUSD (P&R)), Office of the Assistant Secretary of Defense for Readiness (OASD(R)), DoD.

**ACTION:**

30-Day information collection notice.

**SUMMARY:**

The Department of Defense has submitted to OMB for clearance, the following proposal for collection of information under the provisions of the Paperwork Reduction Act.

Consideration will be given to all comments received by January 26, 2018.

*Responses per Respondent:* 1.

*Annual Responses* : 280.

*Average Burden per Response:* 90 minutes.

*Annual Burden Hours:* 420.

*Needs and Uses:* The information collection requirement is necessary to establish a baseline assessment of readiness requirements and available resources to support these requirements at a state level, as well as to identify current gaps in resources for Service members across all components.

*Affected Public:* Individuals or households, Business or other for profit; Not-for-profit institutions, State, local, or tribal government.

*Frequency:* One-time.

*Respondent's Obligation:* Voluntary.
