**AGENCY:**

Office of the Under Secretary of Defense for Acquisition and Sustainment, Department of Defense (DoD).

**ACTION:**

30-Day information collection notice.

**SUMMARY:**

The DoD has submitted to OMB for clearance the following proposal for collection of information under the provisions of the Paperwork Reduction Act.

**DATES:**

Consideration will be given to all comments received by February 5, 2020.

*Number of Respondents:* 1,000.

*Responses per Respondent:* 1.

*Annual Responses:* 1,000.

*Average Burden per Response:* 10 minutes.

*Annual Burden Hours:* 167.

*Needs and Uses:* The information collection requirement is necessary to provide proof of aircraft disinsection to foreign countries that require it, before cargo and aircrew will be allowed to dis-embark in those countries. This standardized form that is used across the DoD satisfies the documentation requirements of disinsection for all 14 countries that currently require it.

*Affected Public:* Individuals or Households.

*Frequency:* On occasion.

*Respondent's Obligation:* Mandatory.
